Hazardous Waste Pickup Request

PRINT A COPY of the waste submission label and attach it to your hazardous waste container!

If any of the following are true, the hazardous waste container will not be picked up by EHS:

1. Unlabeled container or any parts of waste labels unreadable
2. Haz Waste Tag incomplete/ not legible (if applicable)
3. Container not at least 75% full
4. Leaking, corroded, or damaged container
5. Container missing caps, not closed
6. Incompatible container
7. Visibly dirty container exterior

Pictograms are optional in your hazardous waste submission form. The "container type description" could be a 1 gallon glass container, 50 gram bottle or 1 lecture bottle as an example.
